H/C PRI Assessments

One of our certified nurse assessors can come to your home to perform screening for placement into a nursing home through the Hospital and Community Patient Review Instrument (H/C PRI). She will perform a thorough evaluation of the client, determine the level of care required and fill out the necessary paperwork.
